Untimely Submission

Definition:

A medical claim must be submitted within the time frame given by the insurance company or the claim will be denied.

Application:

It is becoming a more common occurrence that health plans are limiting the amount of time allotted in which they will accept claims for payment. This becomes an issue if the original claim is sent to the wrong health plan to pay. Health plans are also becoming reluctant to waive this clause that is usually 60 or 90 days.
