Case Management

Definition:

A method by which a health plan attempts to control costs by directing all of the procedures for care of an individual through a nurse or other health care professional.

Application:

A case manager receives cases of patients who have the potential for incurring high expenses. The case manager, who usually works for the health plan, coordinates the care, while attempting to negotiate discounts on the cost of care.
